Verification of status, on employer or program letterhead, should be sent to the UIC Department of Urology by email attachment, fax, or mail. (See "Contact" for address and fax number)


All activities will be held at the Silverado Resort and Spa. Preferred rates for attendees have been confirmed. Guests are responsible for making their own reservations, in accordance with deadline, payment, cancellation, and other policies established by property management.

Your registration is not complete until your registration fee has been received. Be sure to make your registration fee payment by August 4, 2017 to avoid a late fee. Golf rates will be the same, regardless of when you make your purchase.

In the event you find it necessary to cancel your meeting registration, a minimum $75 processing fee will apply. Requests for cancellation received after August 23, but on or before August 28, 2017, will be subject to a deduction of $150. No registration fee refunds will be issued for requests received after August 28, or for non-attendance.
